Informed Consent for Online Career Counseling

January 4, 2011 by Kate Anthony

All therapists and counselors offer a formal informed consent process, right?….

Why surely, that was a resounding “YES!”

I am often asked if I can supply a sample informed consent for therapists and other helping professionals who are beginning to offer distance services via technology.

The informed consent I use is geared toward mental health counseling and psychotherapy.

My informed consent form is readily available for review at my website. I make the form available on my website:

http://www.mentalhealthonthewebblog.com/informed-consent/

But what about career counseling? Download the form below to view a sample agreement that can be adapted for use by career counselors.

Be sure to obtain legal consult before adapting any existing informed consent forms.
